1. To schedule your initial evaluation, call: 828-683-6900 or use our confidential form to Ask Us to Contact You.
   Let us know if you want the In-Office or Distance (by telephone) evaluation.


2. Download and print both the AIT Evaluation Form and Financial Forms at: Download Forms.
   
   If you prefer, you can ask us to mail the forms to you. Make your request for forms by mail by telephone or by filling out the form at



3. Complete the forms and send them to us either by fax or mail.
